Head of School Update (Required Health Questionnaire)

Dear SSIS Faculty and Staff,

We are cooperating with the Vietnam Ministry of Health and the Department of Education and Training in the collection of health information for all faculty, staff and students prior to reopening school.

This situation requires early and effective detection of suspected cases in order to limit the risk of exposure to others. For this purpose, we count on your kind cooperation and respectfully ask you to complete the following health questionnaire to the best of your abilities. If you answer “Yes” to any of the questions, please provide additional information such as dates, locations, etc. Please complete this survey before Thursday February 6th, 5:00 PM.

Although we already received this information from those families who visited China, we need you to complete it again on this official form.

Any changes to your family’s travel status will require you to complete the survey again whenever you travel until the Coronavirus outbreak is no longer a concern.

A printed survey will be available for those who wish to complete it in Vietnamese tomorrow from HR.
